Reporting Relationship
The Jordan’s Navigator will report to the Director of Education.

Position Summary
The Jordan’s Principle Navigator will plan, organize, implement, coordinate, and evaluate activities related to families with children with complex needs. The Jordan’s Principle Navigator works in collaboration with families, providing advocacy and assisting in navigating the Jordan’s Principle application process. The purpose is to provide program supports to M’Chigeeng First Nation (MFN) families with children living with special needs and to help enhance the child’s life and facilitate health care interventions and developmental stimulation without denial, delay, or disruption. The navigator will also participate in committees/groups in support of coordinating programs and services to M’Chigeeng First Nation members.

Essential Functions

  • Provide and locate needed supports for families with children with complex needs. Services provided may include respite care, crisis intervention, behavior management, and life skills training, whether on a one-to-one basis or in a group environment.
  • Identify and document needs and services for children who require services.
  • Support families, if required, planning for recreational, social and/or educational activities for children and their families is needed.
  • Develop and update resource list.
  • Assist individuals, families, and community to identify and handle denials, delays, disruptions, of public services ordinarily available to children and families with special needs or needs not being met at home or in the community.
  • Assist families (on and off reserve) to complete and submit applications for funding to the Department of Indigenous Services Canada.
  • Conduct assessments through telephone or in-person to confirm eligibility and determine appropriate services based on the needs of the child.
  • Document a coordinated intake assessment that includes referral, assessment, treatment/admission, discharge (if applicable), transition and ongoing care protocols.
  • Communicate the service plan to the family and appropriate Indigenous community organizations.
  • Provide navigation services with a cultural safety and child centered lens, using a trauma-informed and culturally competent approach.
  • Build partnerships with healthcare, school and childcare staff and other professionals.
  • Provides presentations and education programs for the community, families, groups, and individuals as requested.
  • Work with the finance department to ensure that Jordan’s Principle claims are processed and managed accordingly to M’Chigeeng policies and the funding agreement.
  • Support data collection, and write reports required.
  • Monitor and ensure compliance with funding agencies; and ensure preparation and timely submission of narratives and statistical and financial reports when required.
  • Prepare and identify current and future annual budget requirements for the program and ensure maintenance in the levels of funding for program sustainability.
  • Ensure utmost confidentiality and safe keeping of all files and records.
  • Other duties as assigned.

Financial & Reporting

  • In collaboration with the Program Supervisor – Jordan’s Principle program monitor the annual financial plans for the program, individual and group approved financial claims for children and youth involved in the program.
  • As a member of the team, review financial reports monthly from Central Finance and prepare a financial analysis and planning for the program in accordance with the approved budgets

QUALIFICATIONS & REQUIREMENTS

  • Diploma or degree in a Social Services or Health Services or related field ad experience in the child welfare or social services sector.
  • Knowledge of Jordan’s Principle initiatives, provincial health services, the Ontario, and the Child and Youth Family Services Act
  • Experience working in a First Nations (FN) community or organization.
  • Knowledge of First Nation governance matters and protocols.
  • Microsoft Office experience including Excel, Word, PowerPoint, Publisher, Outlook
  • Valid Driver’s License and reliable vehicle.
  • Excellent interpersonal and communication skills; combined with the ability to negotiate positive results while managing positive relationships.
  • Knowledge of First Nation issues, culture, traditions, customs and practices and understanding of First Nations Child & Family matters and interests.
  • Ability to maintain and promote strict confidentiality
  • Well organized and able to multi-task, work well under pressure and prioritize work.

CRC/VSS

Criminal Reference Check (CRC) Rationale:  M’Chigeeng First Nation is in a position of trust to its band members and is committed to providing a safe and secure work environment.  Applicants must have record clear and/or dated of convictions relating to fraud, drugs, assault and theft.  The CRC must be current (30 days) and must be an Original.

CRC/VSS Rationale:  M’Chigeeng First Nation and its agents are in a position of trust in services to band member children and/or vulnerable adults, so a safe and secure working environment is mandatory.  Applicants must provide a clear VSS Certificate and the CRC must have a record clear and/or dated of any convictions relating to alcohol, drugs, assault, violence and theft.  The CRC and VSS must be current (30 days) and must be an Original.
