Position Summary
The Home & Community Care Nurse will be responsible for the implementation and provision of culturally appropriate, quality and compassionate home & community nursing care to M’Chigeeng community members. This will include the assessment, planning, treatment, referrals, health teachings, monitoring and case management of home & community clients.

Reporting Relationship
The Home & Community Care Nurse works under the direction of the Nursing Supervisor.

Essential Functions
An active member of the Health Services team in the delivery of comprehensive Home & Community Care Program comprised of such activities:

  • Utilize effective interpersonal communication skills in providing objective assessment using appropriate assessment tools while planning care with client and family to develop goal based care plans including program discharge that will encourage client independence and participation of client, family and others in the care and rehabilitation of the individual
  • Work in collaboration with the Nursing Supervisor/Home Care Program Manager to complete ongoing assessment of clients to determine their continuing eligibility for services, assess appropriateness of service delivery, readjust goals as necessary and coordinate discharges once wellness goals have been attained
  • Provide appropriate referrals to other service providers as required
  • Provide acute, chronic and palliative nursing care to clients in their home and/or community health centre including but not limited to wound management, IV therapy, palliative care, and case management
  • To provide collaboration with local hospital and/or other care facilities to coordinate and support client care for discharge to community.
  • To participate in relevant nursing, Health Services, and community partner meetings
  • Provide information and heath teaching to clients, family and/or caregivers on self-care, preventative, rehabilitative and comfort measures
  • Coordinate the use of services, supplies and equipment through reports from providers, care reviews and case conferences to ensure an appropriate level of quality care through consultation with service providers and clients; and, to ensure a cost effective use of resource
  • Participate in ongoing professional development to ensure skills are current and relevant to position
  • Participate in caseload reviews with Nursing Supervisor/Home Care Program Manager, Personal Support Workers and other Home & Community Care Nurses to update client status and coordinate client care requirements
  • Completion of all relevant home & community care forms, assessment tools and medical record documentation as per best practice guidelines
  • Provide relief nursing services to the Community Health program as needed, and participate in weekend rotation to provide essential home & community care.

Qualifications

Preferred
Previous work experience in First Nation community and home care nursing would be an asset

Minimum
Baccalaureate degree or diploma in Nursing (Registered Nurse or Registered Practical Nurse)
Valid and current registration with and member in good standing with the Registered/Registered Practical Nurses
Association of Ontario (RNAO/RPNAO) and the Colleges of Nurses in Ontario (CNO)
Valid HCP CPR and First Aid Certificate
Valid Ontario Driver’s License and own transportation

Other
Must have knowledge and/or experience with the culture, history and customs of the Anishinaabe
Understanding and fluency of the Ojibway language would be an asset or a willingness to learn
Criminal Reference Check and Vulnerable Sector Screening (CRC/VSS) requirement

CRC/VSS must be thirty (30) days current, original or certified copy. MFN is in a position of trust and is committed to providing a safe and secure healthy work environment.
